Please note:
- The date when you examined the patient for the purpose of writing this report should be less than 3 months ago (from today's date).

If important information such as clinical history or examination findings are set out in an attached document instead of being entered into this online form, the
attached document must be uploaded via the Supporting Documents page or the medical report would be rejected.
Please make sure the following information is available to you as you will need to enter this information:
- Applicant's ID Number
- Patient's Name and ID Number
It would be useful if you have the Applicant's Email Address as the system will then be able to notify the Applicant that your medical report has been submitted.
Please note that applicants will be able to start filing the deputyship application after the medical report is approved. An email will be sent to applicant's email upon approval of medical report. This may take 2-4 weeks. Please contact the MCA hotline at 63257619 if you do not receive the email notification after 4 weeks.
