Please note:
- The date of examination of the patient for this medical report should be less than 3 months before the date of the medical report.

The following document needs to be uploaded via the Supporting Documents page or the medical report would be rejected:

- The doctor's medical report (which must be exhibited in an affidavit signed by the doctor before a Commissioner for Oaths).
Please note that applicants will be able to start filing the deputyship application after the medical report is approved. An email will be sent to applicant's email upon approval of medical report. This may take 2-4 weeks. Please contact the MCA hotline at 63257619 if you do not receive the email notification after 4 weeks.
